"By making the donation, I certify that this is my personal credit card not funded by a corporation or other prohibited source."
"This contribution will not be reimbursed by anyone else and will not cause my contribution to this candidate to exceed the $500 per individual contribution limit to a Wisconsin political campaign."

Required Information for Contributions:
1. The date, full name, and street address of each person who has made a contribution to the candidate committee, together with the amount of the contribution. WIS. STAT. § 11.0204(1)(a)1.
2. The occupation, if any, of each individual contributor whose cumulative contributions to the candidate committee for the calendar year are in excess of $200. WIS. STAT. § 11.0204(1)(a)3.
